All-on-4 (Full Arch, 4 Implants)

The standard All-on-4 protocol uses four implants per arch — two straight in the front and two angled in the back. This distribution maximizes available bone and allows for immediate loading of a provisional prosthetic the same day as surgery in most cases. The final permanent prosthetic is placed after osseointegration.

All-on-6 (Enhanced Stability)

For patients with good bone volume or those requiring a more complex restoration, All-on-6 uses six implants per arch to distribute load further. This option is appropriate when additional stability is needed or when the prosthetic design benefits from more anchor points.

Implant-Supported Overdentures

For patients who prefer a removable option while still gaining implant stability, overdentures snap onto implants placed in the jaw. Unlike traditional dentures, they don’t rest on the gums — the implants hold them in place and help prevent bone resorption over time.

Immediate Load vs. Staged Protocols

Immediate load protocols allow a temporary prosthetic to be placed the same day as implant surgery. Staged protocols involve a healing period of 3–6 months before the final restoration is attached. Dr. Nicholas Goetz holds a DMD and MS from the University of Florida, completed his Prosthodontic Residency at UF, and completed a Maxillofacial Prosthodontics Fellowship at UCLA — training focused on complex oral and facial reconstruction.

He is one of approximately 150 board-certified maxillofacial prosthodontists in the United States, with clinical experience from U.S. Army and VA Medical Hospital settings. For All-on-4 patients, that means every stage of treatment — from implant planning to final prosthetic design — is handled by a specialist trained to manage the most complex cases.

Consultation & CBCT Imaging

Dr. Goetz begins with a comprehensive evaluation including CBCT 3D imaging to assess bone volume, density, and anatomy. This determines whether you are a candidate for standard All-on-4, All-on-6, or whether preparatory procedures are needed. You’ll discuss your goals, timeline, and restoration preferences at this stage.

Implant Placement & Provisional Restoration

On surgery day, any remaining failing teeth are extracted and the four (or more) implants are placed. If bone and implant stability allow, a temporary full-arch prosthetic is attached the same day so you leave with functional teeth. You’ll be on a soft diet during the healing phase while osseointegration occurs.

Osseointegration & Healing

Osseointegration — the process by which titanium fuses with your jawbone — takes 3–6 months and cannot be rushed. During this phase you’ll wear the provisional prosthetic. Dr. Goetz monitors healing at follow-up visits to confirm the implants are integrating properly before moving to the final restoration.

Final Prosthetic Placement

Once osseointegration is confirmed, the temporary prosthetic is replaced with your final custom-fabricated restoration. The final prosthesis is designed by Dr. Goetz to match your bite, facial proportions, and esthetic goals — engineered for long-term durability and a natural appearance.

Good candidates have sufficient bone volume to support four implants, are in generally good health, and have realistic expectations. Patients who have lost significant bone may still qualify — the angled implant placement in All-on-4 often eliminates the need for bone grafting.
